Auto Responder: "failed to open stream", help? :)

Hello

I am having some difficulties setting up an auto-responder.

I keep getting 2 different kinds alert message in my e-mail saying:

When I don't press 'I want a confirmation e-mail' and don't enter the verifyimg.php characters, I get this:

The following error occurred in FormMail :
Failed to open template "/template/plain.txt" : failed to open stream: No such file or directory

I do still receive the complete form though in the webmaster mail.

When I DO enter the verifyimg.php characters, I don't receive the e-mail and I get this error:

"The following error occurred in FormMail :
Autoresponder failed"

The form I am trying to submit is: http://www.samsfootball.com/picks14.html

and my form mail is http://www.samsfootball.com/Submit.php

If anyone could figure out why I keep getting these errors, I will be forever in your debt Thank you!!
