This URL is a Form ...error( I am sure it my error but....)

I am receiving this message when I try to submit the sampleform.htm. The test works fine.

It appears the form is not working correctly as there was no data found.
You're not supposed to browse to this URL; it should be accessed from a form.

I am sure you have seen this 100 times but I have had problems with three other php forms on this site and I am starting to wonder. Could be a situation with the server?

Just an update,

I used the configuration wizard as I did the last form. Uploaded it tomy own site and it worked Great!

Would this have someting to do with the FromAddr and such?
