FormMail can work as an interface to a system that works with a MySQL database.
TectiteCRM is our CRM system based around MySQL. FormMail already knows how to interface with TectiteCRM and hence the MySQL database underlying it.

I would also like to have formmail use a mysql database. Can I have formmail take the form info and put it into a database then send me an email to notify me that there is a new entry in the database? How can I configure it to do this?

If it's a database of your own design, then you'll have to write PHP software and add it to FormMail.
If you use TectiteCRM, then it can send to MySQL right now.

Does it matter where I put the code? Can I just append it to the end of formmail.php?

Yes it does matter, but it will probably be fine to just append it to the end.
FormMail only gets to the end once everything has worked.
The redirect of the user's browser should be OK too, but you'll need to test thoroughly. If it's a problem, then you can put the code in a function and call that funciton just before the Redirect.
