I have a survey page that i am trying to get to work, but when you fill out the survey and hit submit all you get is the FORM MAIL document, it is supposed to send the survey results to my boss but doesn't. any ideas?

What do you mean, "all you get is the Form Mail document"?

Do you get any error messages? Do you receive an error to the email set in $DEF_ALERT? Did you set $DEF_ALERT? Does your host support php? Did you use the "Configuration Wizard" provided by Tectite?

Can you provide a link to the form in question?
