Formmail Script Error

I was able to post my form, it sendd my results via email but it also sends an additional results email with this error message:
The following error occurred in FormMail :
> Unable to create check file "/fm54089d.txt": fopen(/fm54089d.txt) [<a
> href='function.fopen'&gt;function.fopen&lt;/a&gt;]: failed to open stream: Permission denied

I can't find any information about this text file in the forums... can anyone help?
