It could be a number of things. The first thing to do is to receive the alert message by setting DEF_ALERT.
If FormMail cannot process the template it should be sending you the exact cause in an alert message.

Hi, I've just set up formmail.php and it works like it should, BUT I'm having trouble getting it to use an HTML template.
I have specified it in the form using
<input type="hidden" name="mail_options" value="HTMLTemplate=template.htm,TemplateMissing=N/A,SendMailFOption" />
and have directed formmail.php to the appropriate URL for templates (also tried directory but no joy).
It works fine without the HTMLTemplate=template.htm,TemplateMissing=N/A, part but no template.. as soon as I add it in I get the "An error occurred while processing the form." screen.
Is there something I'm missing?
