Same formmail.php in two different forms

This is maybe a stupid mistake I am making but I am using the same formmail.php for two different forms. The form is working fine in one form but in the other form is not. The form that doesn't work appears like everything is working fine but I don't receive the email results.

The form that is working IS on this page:

What am I doing wrong?

Never mind. I figured it out.

For some reason my spam filter was filtering the messages from one form and not the other. Now I feel silly.
