Form works great until.....

I created a form and when I try to change the layout of it, basically adding to it, it won’t send to me. Any chance someone could take a look at it and let me know what I’ve done wrong?

The form is at http://www.paecarclub.org/signup.htm and I used a signup.php which is in the root directory.

I stand corrected, it doesn't work even uploaded as is with no changes what so ever. I did a contact form and I put all the forms and the .php file in a folder called contact and it doesn't work. I'm at a loss!
