formmail uses a special field name called "email" ur form must have that or derive it from a field it already has to collect the email address
it then sets the From address on the email to the "email" field (so u can reply to the email).

Hello,
I need help troubleshooting a problem with my FormMail Script and possibly my form. I am happy to report that the form works and I'm getting no error messages. However, the recipient of the form data is reporting that when a user enters an email address that is not being reported to the client. He has sent me the emails he gets from the form submissions and everything is there but the email field and the data that users of the form enter for their email addresses.
I can send the HTML for my form and my PHP file for the configuration if you'd like.
Thanks
Mark
