Required fields not sent

I have a form with several required fields. Some of the required fields are not included in the email I get. I've stared at the code and can't figure out what is wrong. Here's the page:

"email" is excluded by a "mail_options" spec, but the rest should be OK.

Thanks! I fixed email (no idea how or when I did that!). The subject is not coming through.
