Can anyone offer any assistance? I am having two issues with my form.

1) I am not receiving any emails
2) I am receiving the following error when completing the form:

An error occurred while processing the form .

The staff at www.hostname.com have been alerted to the error .
www.hostname.com apologizes for any inconvenience this error may have caused.

I am experiencing these problems and I am only using the sample form and script at the moment.
