Followed Instructions - Still not sending email

Hello, I feel I followed the instructions properly. I am not receiving any emails from the form. I am not really sure how to troubleshoot this or where to look. Any help is appreciated.

It's working now. Don't remember what I did, made several changes, double checked all settings. Works fine now. Thanks.

Could you possibly send me a copy of your working script? I'd appreciate it so much. Mine isn't working and I've REPEATEDLY downloaded the script and followed the instructions to the tee. It used to work and then suddenly stopped.
