Not Receiving Email

The script was installed and configured, as per the instructions on the site. Loaded the script onto the server, as well as the test form provided with the script download.

Tested the script using the test form and received email honkey-dorey, aside from it being sent to my spam folder. No biggie. However, when running the script from the form I created, the email does not go through, to my spam folder, or my inbox. The thank you page is displayed, assuring me that my message has been sent and that everything worked okay.

I've been banging my head against this for a while now and am finally willing admit defeat and ask for professional help.

Hi,

Your field names aren't correct:

<input type="text" name="E-mail Address|at:email|r|Please enter a valid e-mail address." id="intakeFormIEmailAddress" class="validate email" /> 

This field name should be "EmailAddr", not "E-mail Address|at:email|r|Please enter a valid e-mail address.".

Please use the "Convert a form" feature in the Configuration Wizard to ensure your fields are valid.
