Is the email you're receiving a form submission or an alert message? You can tell by reading the first few lines of the email and the subject line.

I've gone through the 'How-to' guide for creating a customized PlainTemplate but there's no change in the email I receive.
I checked the source code of the form page and it accurately identifies the mail_options: <input type="hidden" name="mail_options" value="PlainTemplate=PlainTextEmail.txt,TemplateMissing=n/a" />
I have used the $TEMPLATEDIR (complete path to the file) and also tried the $TEMPLATEURL (referencing the location on the web server).
I have uploaded the updated formmail.php, the PlainTextEmail.txt, and the updated form pages.
I do not get an error, I just get the same old content in my email. Ideas?

It's a form submission. It has the message subject 'More Info Request' , followed by the message as taken from the form, then the following fields:
REMOTE_HOST
REMOTE_ADDR
HTTP_USER_AGENT
REMOTE_USER
