do you have already read (about) line 2972 - 3026 of formmail.php?
(it's about SENDMAIL_F_OPTION and INI_SET_FROM)
normally it should work, otherwise
just found, maybe of interest to you:

Welcome to the Tectite Forums! You can download and get support for our free PHP FormMail (form processor) and other free software.
I'm fairly familiar with this script and have used it on many sites before. This was my first try with both multiforms & fmbadhandler. And everything works!
Except.... The resulting email I get is sent from the server, not the address in the email field. I set the SENDMAIL_F_OPTION to true and I also uploaded testmail3.php to see if it was a server problem. The test worked ok but my email result remains the same. I am not using the mail option 'FromAddr'.
Any ideas?

Thanks for your response. I had come across that thread & tried everything so I ponied up the cash and got some help directly from Russ. The resulting fix is in the last post in the thread you've linked to. The short version is that to work around the fact that the script wouldn't keep data for the two fields email and realname, I had derived them which solved that problem but created this one. To solve, we substituted the value from one field into the corresponding one instead. It works now.
oh
perhaps I should have read the whole thread.. but I were too lazy
