    I have a form at http://demos.onlineacc.com/fitnessfactory/survey.html

    There are two fields on the page. It is the section called "Primary Membership Location"

    under this section is Palisades Park, NJ and Rockaway, NJ. Currently I have an issue that someone can click both radio buttons and they will become active. I need to find out how to stop that so that if they choose Palisades Park, NJ they cannot check Rockaway, NJ as well.
    The reason why i am posting here is this: I want the form to be able to email one individual in the office if they choose Palisades Park and if they choose Rockaway the email will go to two other individuals. In Palisades Park there is one person that has to get the emails and in Rockaway two people have to get a copy of the emails. Can anyone direct me?

    radio buttons are related by having the same name

    ur radio buttons have different names. so the browser thinks they are two separate sets of radio buttons, and each set has one button

    I know this thread is quite old, but that code was exactly what I needed on my form. I used radio buttons to allow the user to select who there massage would go to. Thanks.

    This awesome. I cannot wait to test it out.

    I am wondering, if you check more than one box, will it send the form to each recipient?

    For example, if I check box 1 and also box 3, will it send to the recipients designated for both box 1 and box 3?

    I still need to set up a test form for this and might answer my own question soon.

    Hi,

    I wouldn't put the email addresses in the form at all. Use the method I described in this post:

    except you'll be using checkboxes instead of a dropdown. Just make sure that all of your checkboxes are named "recipients[]" (note the brackets) if they can select multiple recipients. (If you only want them to select one recipient, then use a radio button instead and just name it "recipients" ... no brackets.)

    Checkbox Example: (for multiple recipients)

    Code:
    <input type="checkbox" name="recipients[]" value="John"> John Doe
    <input type="checkbox" name="recipients[]" value="Mary"> Mary Smith
    Radio Button Example: (only one recipient)

    Code:
    <input type="radio" name="recipients" value="John">
    <input type="radio" name="recipients" value="Mary">
    The names John and Mary (and any others) along with their email addresses, will go in your INI file as described in the link above. When the form's submitted the script will look to the INI file to see what email address goes with the selected recipient names.

    Are you asking if you have one checkbox, how to make that one checkbox send to multiple recipients?

    No Sorry i wasn't more clear. i meant how do i send to multiple recipients with multiple checkboxes.

    Okay...am doing what you want here I believe...http://www.hellerservicecorps.org/volunteer-sign-up.php

    It took some fiddling around, but once I pieced together the hints up here, I finally got it to work and it works great.

    1. I created derived fields
    <input type="hidden" name="derive_fields" value="recipients=
    %if(Education1;Education1;%''%)%.%2C%.
    %if(Major_Events3;Major_Events3;%''%)%" />

    2. I set up my boxes in my form:
    <input type="checkbox" name="Education1" value="Yes Book Buddies"> Book Buddies
    <input type="checkbox" name="Major_Events3" value="Yes On-Site Service" /> On-Site Service

    3. I set up an ini file to hold the recipients (this is defined in the form.php file at $FORM_INI_FILE. Name is whatever you want with .ini extension. The .ini file contains the following:
    [email_addresses]
    Yes Book Buddies = "1stemail@email.com"
    Yes On-Site Service = "2ndemail@email.com"

    4. I put all the acceptable e-mail addresses at $TARGET_EMAIL

    I believe that was all. Try a sample like above to see if it works and if not let me know.
