Problem deriving fields in formmail

But, I am getting a reply from a nobody@etc.etc.com address. Also, the top lines of the message area are:
Below is the result of your feedback form. It was submitted by
() on Tuesday, May 08, 2007 at 08:45:19

seeing this:

derive_fields: email = EmailAddress, realname = FirstName + LastName

in ur form results means youve got some typo in ur form
