Need directions to make a questionnaire form

I would like to learn how to make a questionnaire which I can send to my customers please as a plain text document.

in the abstract with html it could maybe work, because you are able to install formmail.php on one server and use it from different webspaces, but i am not really sure if it will be working with a html-email

as plain text i think it certainly won't work

but why won't you send your customers an email with a link to your questionnaire form which is online somewhere?
so you create a simple form with your questions and so on..
